Hearing and Vision Screening now offered

 

Hello CAVA families,

CAVA is pleased to announce free vision and hearing screening for our students.  
Each screening will take approximately 15 minutes.

The following locations and dates available for December:
12/4/12- San Jose
12/5/12 Pleasant Hill
12/6/12 San Jose
12/12/12 Sacramento
12/13/12 Sonoma
12/17/12 Yuba City
12/18/12 Clearlake

Please read the directions on the website and select a location by clicking on the box (when clicking the highlighted name of the city being offered you will see the address and a Google map).
This will allow you to schedule an appointment on the day(s) that the site is available.
Please remember to fill out the form prior to your appointment.  This can be downloaded from the website.
We will be offering more locations, days and times after the holidays.  We will post those times in the next two weeks.
